Voodoo Pin is an active item.

Hurts Isaac without actually damaging him, activating any items that would trigger upon taking damage.

Using Voodoo Pin while having 9 Volt and Habit only charges the item by 1. This is possibly to prevent players from having infinite Voodoo Pin uses.
It will kill Isaac if he has all his heart containers filled with rotten hearts (possibly a bug).
It will kill Isaac if used against The Witness unless he also has The Polaroid.

Habit: No effect.
Whore Of Babylon: Activates Whore of Babylon's effect for the current room regardless of red hearts available.
Perfection: Destroys Perfection.
Polaroid: Activates Polaroid shield regardless of red heart amount.
Negative: Activates Negative effect regardless of red heart amount.
Bloody Lust: Every usage increases the character's damage.
Holy Mantle: Does not remove the shield.

This item has a counterpart in the Afterbirth† DLC called Dull Razor, which is identical to Voodoo Pin except in appearance, name, and certain interactions.
Voodoo Pin was one of the few items not imported into Repentance, alongside Book Of Despair, Bowl of Tears, Donkey Jawbone, Knife Piece 3, Menorah, and Stone Bombs.
