Smoky is an alternate version of Death's Head that bounces around the room with a purple aura. If Isaac touches an aura, he will briefly become afraid, unable to fire tears for a short period. It dies when the room is cleared.

Trivia

Smokys are a reference to Blue Bubblesfrom The Legend of Zelda, wherein some of the games Blue Bubbles curse the player, making them unable to attack or use items.
