- Upon taking damage the first time in a room, the familiar leaps from Isaac's head, targeting an enemy and landing on them, dealing damage and releasing a rock wave. For the rest of the room, the familiar targets and attacks enemies similar to Dead Bird.

- The name of the item is a play on words. Rather than an actual birdcage, the item resembles a small, birdlike version of the boss named The Cage which shares the boss's general appearance and behavior of slamming down on enemies and creating a rock wave upon impact.
- The exact type of bird which the item is based on is unclear, but it seems likely to be a turkey or chicken based on its large body and tiny limbs (though this might just be to make the item's appearance more similar to that of The Cage).
